Toenail Fungus (Onychomycosis)

Why It Happens

  • Organisms: Trichophyton rubrum > 80 % of cases; non-dermatophyte molds & yeast less common.
  • Risk Factors: Damp socks, tight shoes, prior athlete’s foot, nail trauma, diabetes, immunosuppression.

Signs & Symptoms

  • Nail thickening, yellow/brown streaks, chalky debris, onycholysis (lifting), occasional odor or pressure pain.

Diagnosis

  • KOH microscopy & culture (in-office scraping).
  • PAS stain if culture negative but suspicion high.
  • Dermatoscope for longitudinal spikes or ruin-appearance.
